Paperwork and Forms
Please bring a photo ID card along with cards from any insurances you will be using.
We need you to fill out a few new patient forms before your appointment, and you can find them on our Forms page. If you can’t complete them before your visit, please plan to arrive 15 minutes early – we want you to get the most out of your time with Dr. Seger!
Your First Appointment at Our Aston, PA Dentist Office
Upon your arrival, our receptionist Linda will welcome you warmly by name. You can complete any necessary paperwork in our comfortable reception area.
When it’s time for your appointment, Tracy will show you back to your room and help you get settled. She will take x-rays if you don’t have current ones, and she will also take intraoral photos and document the condition of your teeth.
Dr. Seger will introduce himself and chat with you for a few minutes to get to know you. He will review your information, evaluate your teeth and gums and perform an oral cancer screening. Based on his exam, Dr. Seger will explain any issues that need attention and work with you to develop a plan for addressing them.
Before you leave, Tracy will review your treatment plan, discuss your insurance benefits, and explain our financing options. We do everything possible to make sure that your personalized plan fits your needs in every way.
